A randomized, open-label study to assess the efficacy and safety of inhaled insulin treatment was conducted by investigators from the University of Vermont College of Medicine in Burlington, Vermont; the University of Miami Medical Center in Miami, Florida; and Pfizer Central Research in Groton, Connecticut, all in the United States. Twenty-six patients with type 2 diabetes underwent a screening visit, a four-week baseline phase and a 12-week treatment phase. Treatment consisted of inhaled insulin before every meal with a bedtime injection of ultralente insulin. Patients performed home glucose monitoring and their insulin dose was adjusted weekly in accordance with a target preprandial plasma glucose level of 5.55 to 8.88 mmol/L. Results showed that glycemic control significantly improved after inhaled insulin compared with baseline. The treatment was well-tolerated and pulmonary function tests detected no adverse effects after three months. "Larger-scale studies are ongoing to provide long-term efficacy and safety data," the investigators report. Ann Intern Med 2001; 134: 203-207.
